Excellus BlueCross BlueShield recognizes 2025 Circle of Excellence Award winners

SYRACUSE, NY – Excellus BlueCross BlueShield has announced the recipients of its 2025 Circle of Excellence Awards, recognizing healthcare provider organizations that have demonstrated exceptional performance in clinical quality and outcomes.
The Circle of Excellence Award recognizes provider organizations that deliver outstanding care and achieve strong results on key health quality measures. To qualify, a provider group must perform at a very high level—ranking in the top 10%—in at least five out of six important areas of care. From those high-performing groups, award winners are selected based on their overall performance across all measures, using health plan data.
The following organizations achieved the highest performance rates across the greatest number of measures within their respective lines of business:
- Safety Net Top Performer: CNY Family Care
- Commercial Top Performer: CNY Family Care
- Medicare Top Performer: Internal Medicine Associates of Auburn
CNY Family Care
Recognized as the top performer in both the Safety Net and Commercial lines of business, CNY Family Care is a primary care practice based in Central New York that provides comprehensive, patient-centered care for individuals and families. The organization focuses on preventive services, chronic disease management, and improving overall health outcomes in the communities it serves.
Internal Medicine Associates of Auburn
Recognized as the top performer for the Medicare line of business, Internal Medicine Associates of Auburn is a physician practice serving the Auburn community, specializing in adult primary and preventive care. The organization is dedicated to managing complex health conditions and delivering coordinated, high-quality care.

Performance Measurement
Awards are based on calendar-year performance and evaluated by line of business using health plan quality reporting. Core measures include:
- Glycemic status for patients with diabetes
- Eye exams for patients with diabetes
- Controlling high blood pressure
- Breast cancer screening
- Colorectal cancer screening
- Statin therapy for patients with cardiovascular disease
Top performers are identified based on comparative performance rates among eligible provider groups meeting the 90th percentile threshold.
